If your previous passport is lost or no longer available, then you must provide a signed and dated letter to the Chinese Consulate, Visa Section, explaining what happened to your previous passport. This rule applies to both US, and non US Passport holders.
• Fully Completed Visa Application Form. Please note: The fillable PDF application form must be typed in CAPITAL LETTERS. Every question must be answered (please answer NONE if the question does not apply to you). Improperly completed Chinese visa application forms may delay the processing of your Chinese visa.

• Copy of airline tickets or flight itinerary.
• Copy of hotel reservation.  If staying with friend or family, then an invitation letter and copy of the Chinese ID card of the host.
• Copy of Drivers License or Valid State ID
• For Non – US Citizens: Original Green Card or proof of Legal Status in United States, plus Copy of travel itinerary or airline tickets is required. Please be advised that non-US Citizens will ONLY be issued Single-Entry Visas.
• For Minors: A copy of the Birth Certificate and a copy of Parent’s Passport is required.
